Infection

 SCIP-INF 1 Prophylactic antibiotic received within one hour prior to surgical incision

 SCIP-INF 2 Prophylactic antibiotic selection for surgical patients

 SCIP-INF 3 Prophylactic antibiotics discontinued within 24 hours after surgery end time (48 hours for cardiac patients)

 SCIP-Card 2 Surgical patients on a beta-blocker prior to arrival that received a beta blocker during the perioperative period

Thromboembolic

 SCIP-VTE 1 Surgical patients with recommended venous thromboembolism prophylaxis ordered

 SCIP-VTE 2 Surgery patients who received appropriate venous Thromboembolism prophylaxis within 24 hours prior to surgery to 24 hours after surgery

 SCIP-VTE 2 Surgery patient who received appropriate venous thromboembolism prophylaxis within 24 hours prior to surgery to 24 hours after surgery

SCIP INF 1
Time of beginning of ABX admin — Surgical Incision time <60 min

SCIP INF 3
Time of last dose of abx — Time of closure of surgical incision <48 hours

SCIP INF 7
First temp taken within first 15 min. of arrival in PACU is => 36 Celsius
