The Staff and Faculty Assistance Program (SFAP) is provided by AllOne Health (formerly called Perspectives) to assist you and your family members with the challenges of daily living. The program is confidential, available 24/7 and provides the following services:

Getting Started
Log in to the AllOne Health portal by using the log-in button in the upper left corner of this page. To create an account, click the 'Sign-Up' button in the green box and use your UChicago email, the employee access code UNI500, and create your own unique password. Starting in January 2024, all users should create a new account using these instructions, even if you previously had logged in before.
Note: It is extremely discouraged to use any iterations of your CNetID password for AllOne Health. After you create your account, you'll use this new password you've created and your UChicago email for future log-in purposes.

Mobile App
Once you've created your account via desktop, download the AllOne Health App for a smarter, faster way to access your your Assistance Program. The app delivers a powerful mobile experience that puts support in the palm of their hand—making care more immediate, intuitive, and accessible than ever before. You can use your UChicago email and unique password to log-in to the app.

Costs
Use of the SFAP is free. AllOne Health does offer access to short-term counseling, webinars, articles, and other resources at no cost, in additional to free referrals. If you or a family member need outside referral services or a treatment program, Perspectives will assist you in obtaining the best care as reasonably priced as possible, outside of the AllOne Health network. Any costs incurred by outside referral services or treatment programs are your responsibility.

Confidentiality
Confidentiality is an important aspect of the services provided by AllOne Health. Information you discuss with AllOne Health will not be shared by them, except as may be required by Federal and State laws. No record of your participation will appear in your personnel file. In an instance where the referral is work-related, with your written permission, your supervisor may be told whether you have followed through with your appointment.
International Services
International members have access to services through AllOne Health' partner site, Lyra. Use this QR code to access the Lyra site and enter the company code UniofChicago to request services.
